When Depression Doesn’t Respond to Medication

Depression is often considered “treatment-resistant” when symptoms persist despite trying two or more antidepressant medications at adequate doses and durations.

People with treatment-resistant depression may experience:

  • Ongoing low mood or emotional numbness

When medications haven’t helped — or caused unwanted side effects — rTMS may offer another path forward.

What is rTMS?

Repetitive Transcranial Magnetic Stimulation (rTMS) is a non-invasive neuromodulation therapy that uses focused magnetic pulses to stimulate specific areas of the brain involved in mood regulation.

rTMS:

  • Is FDA-cleared for Major Depressive Disorder
  • Does not require medication, anesthesia, or sedation
  • Does not involve surgery or electrical shocks
  • Is performed while you are awake and alert

We deliver magnetic stimulation through a coil placed gently against the scalp. Most patients describe the sensation as a light tapping or knocking feeling.

Why rTMS Works for Depression

Research has shown that depression is associated with altered activity in certain brain networks involved in mood, motivation, and emotional processing. rTMS is designed to help normalize activity in these areas by stimulating underactive regions of the brain.

Large clinical studies have demonstrated that rTMS can:

  • Reduce depressive symptoms
  • Improve daily functioning and quality of life
  • Help some patients achieve remission when medications have failed

rTMS has been studied extensively for over a decade and is now considered an evidence-based option for treatment-resistant depression.

What to Expect From rTMS Treatment

Initial Evaluation

Your care begins with a comprehensive clinical consultation to review your history, symptoms, and prior treatments. As part of this evaluation, we also perform a quantitative EEG (qEEG) to assess brainwave activity patterns associated with mood regulation.

This additional data helps our clinical team better understand how your brain is functioning and supports more precise targeting when planning rTMS treatment.

Treatment Schedule

  • We typically perform sessions Monday through Friday
  • Each session lasts about 30–40 minutes
  • A full course of treatment usually spans 4–6 weeks

During Treatment

You’ll sit comfortably in a chair while the neuro-technician delivers the magnetic stimulation. You can return to normal activities immediately after each session—there is no downtime.

Many patients begin noticing changes in mood, sleep, or energy within the first few weeks, though individual responses vary.

TRICARE Coverage for rTMS

rTMS is FDA-cleared for Major Depressive Disorder, and TRICARE may cover treatment when specific medical criteria are met. At this time, TRICARE is the primary insurance plan we work with for rTMS.

Coverage typically requires:

  • A diagnosis of Major Depressive Disorder
  • Inadequate response to at least two antidepressant medications
  • No medical contraindications to rTMS therapy

Coverage is not guaranteed, but our team has experience working with TRICARE beneficiaries.

    Absolute contraindications for cortical MeRT treatment: Pacemaker, Defibrillator, Vagal Nerve Stimulator, VP Shunt/ Magnetic intracranial shunts, Deep Brain Stimulator, Epidural Cortical stimulator, Steel shunts/stents, Cranial metal fragments (i.e. shrapnel, excluding titanium), Cochlear implant, Aneurysm clips, coils, pipelines flow diversion, Pregnant or breastfeeding, Primary brain cancer / metastatic legions in brain (unless palliative care), Magnetic dental implants, Implanted cardio-verter defibrillators (ICD), Ocular implants, Suicide attempts.
  • Relative Contraindications

    Relative contraindications: These require closer protocol attention and may or may not disqualify someone from receiving cortical MeRT treatment, depending on the doctor’s discretion and the person’s individual condition. These include: History of Seizure or seizure disorder, Titanium shunts/stents, Spinal Cord Stimulator, Hearing aids, Ferrous cortical implants, Magnetic ink tattoo, Bipolar Disorder Type I/II, Baha Implant, Suicide ideation.
